OUR TERMS & CONDITIONS (PLEASE READ)

By submitting this request, you have agreed that this shall serve as your official request for a service provided by ALKATEK, INC. (The “Service Provider”), and that you have completed this form to the best of your knowledge as the decision maker. You understand that this is on-going payment registration and have agreed to make payment as listed and selected (payment for FREE Basic Account Holders is deferred for 3months and account upgraded to STANDARD Premium account). You also understand that you and/or The Service Provider reserve the right to cancel the service at anytime with written notice (Notice by e-mail shall be deemed sufficient Written Notice.) In the event that you choose to cancel the service, no money shall be refunded to you; however a refund transaction shall apply (Monthly fee minus Pro-rated fee and immediate cancellation of any applicable selected value-added services) should The Service Provider choose to cancel its service(s) to you. You have agreed to provide substantial proof of your company active registration and to actively provide any change or updates to you account as needed. Your failure to abide by the terms and conditions hereby set forth will result in the termination of service by The Service Provider, and loss of eligibility to re-register for a period of 12 months.
